Life Planning: Transition to Adulthood


National

Connecting to Success is an electronic mentoring program designed to promote successful transition of youth with disabilities to adult life. This program uses a combination of e-mail and face-to-face meetings to facilitate mentoring relationships between young people and caring adults.

New Sign Disability.gov via their Transition Planning section, provides resources and information to help parents of kids with disabilities work with their children to plan and prepare for postsecondary education, employment or other opportunities.

Heath Resource Center, the online clearinghouse on postsecondary education for individuals with disabilities, has developed an online Parents Guide to Transition. This resource features information, tips and tools parents can use to better prepare their children with disabilities for transition into secondary education, a career or the community.

National Center on Secondary Education and Transition provides information and programs for youth with disabilities in order to create opportunities for them to achieve successful futures. This site provides a directory of state contacts as well as links to other helpful resources.

National Consortium on Leadership and Disability for Youth (NCLD) is a youth-led resource, information, and training center for youth and emerging leaders with developmental disabilities. They develop projects and educational materials to better prepare youth with developmental disabilities for the transition to adulthood. Check out their website or call (877) 871-0744 to learn more about their initiatives in your area.

Technical Assistance Alliance for Parent Centers provides a State Directory of Parent Centers. Parent Centers serve families of children of all ages (birth through 26 years) with all disabilities; physical, cognitive, behavioral, social, and emotional. Services for young adults with disabilities can include housing referral and coordination and employment programs, among others.

The ARC of the United States has chapters throughout the United States. Many of these chapters provide school to community transition programs for youth with intellectual and developmental disabilities, including autism. To find out if your local chapter provides a transition program, contact your Local ARC Chapter.

New Sign Think College is a website designed to educate transition-aged students with intellectual disabilities and their families on the possibility of successfully attending college. More specifically the website features a database of postsecondary education options, a database of articles related to postsecondary education and intellectual disabilities, a calendar of upcoming training events and conference presentations and helpful tools and hints about college for students.

Transcen, Inc. is an organization primarily dedicated to assisting transition professionals. Their website features Self Paced Modules to help professionals, families and youth better understand such opportunities as work-based experiences, youth empowerment, health and social services, and family supports.

National Collaborative on Workforce and Disability For Youth provides information about employment and youth with disabilities. Their website provides information for parents and families, youth with disabilities and the professionals who work with them.

Youthhood is an online community for teens and young adults with disabilities to gather to discuss and plan their futures. The web site is organized using the metaphor of a neighborhood, with each content area reflecting a place in the neighborhood that youth can "visit." For instance, there is The High School, The Job Center, The Community Center, and several other "neighborhood" centers. By visiting the different locations, youth learn about issues related to planning for a successful transition from high school to adult life.
